Extremely high script & extraordinarily high prism

PATIENT PROBLEM

Our patient Anna is extremely myopic or short sighted.

Anna has also started to experience headaches and blurred vision. She feels unsteady on her feet and has regularly lost balance. This is accompanied by dizziness which is impacting her ability to read, hindering her computer use at work, and affecting her overall quality of life. As a result, Anna is suffering from anxiety.

Anna has been diagnosed with Binocular Vision Dysfunction (BVD), also widely known as double vision. This condition is where the eyes struggle to work together to create one clear image.

Not only does Anna have an excessively high script to treat the myopia, the prism required to correct the BVD is also extreme.

WHY IS THIS AN ALMOST IMPOSSIBLE SCRIPT?

Extremely high script: -26.00 & -27.00

Extraordinarily high prism: 11.5 BD / 30.00 BO & 11.5 BU / 30.00 BO

Anna requires correction for extreme myopia -26.00 in her right eye and -27.00 in her left eye. This alone makes it a challenging script.

But that’s not the truly impossible part!

The prism required is the part that’ll likely have your lab saying it can’t be done.

The script: Extremely high script & extraordinarily high prism

As a recap, prism is used to help correct double vision (BVD) by bending or refracting light before it enters to eye to ensure light hits the eye at the correct focal point. The double vision is addressed by aligning the image from both eyes so that the brain receives a single clear image.

The direction at which the light is bent or refracted is determined by the prism’s orientation – whether that prism be base up (BU), base down (BD), base in (BI) or base out (BO).

The prism in Anna’s script has been split into 11.5 base down (BD) and 30.00 base out (BO) in her right eye, and into 11.5 base up (BU) and 30.00 base out (BO) in her left eye.

This makes the prism required to correct Anna’s BVD a total of 23 dioptres up and down. And a total of 60.00 dioptres for base out! This amount of prism is extreme and is impossible to manufacture. Or is it…?

THE IMPOSSIBLE SOLUTION

The impossible solution is a handmade boutique lens from a speciality lab that not only addresses the BVD with extreme prism, but also caters for Anna’s severe myopia.

Normal optical machinery cannot make these lenses for Anna. That’s why your lab is likely to tell you it cannot be done.

The script is so extreme that specialised equipment and a high level of expertise and smarts is required to create the lenses.
